If you are coming in for a counseling appointment, there are documents you’ll need to fill out and required notices you’ll need to acknowledge receiving and reading.
To save time during your visit, you can download and complete these forms before coming in.
Required for Your Visit
You will be required to complete the following forms before your first session at Capital EAP. Completing these in advance of your visit will save you time at our offices.
Please fill out the Client Information Form and whichever initial intake form below is appropriate for your scheduled appointment.
Health Screenings
The Patient Health Questionnaire (PHQ) is a self-administered screening and diagnostic tool for depression, which has been field-tested in office practice. The screening is quick and user-friendly, and assists in recognizing symptoms of depression to facilitate seeking further diagnosis and treatment.
